At SENT, Inc., we believe that everyone deserves a fair chance to thrive. Yet, we know that for many in our community, accessing vital resources can feel like an uphill battle. Complicated forms, confusing eligibility criteria, and limited availability often prevent people from getting the help they need.
That’s where our Case Management Services come in.
Our dedicated team—featuring Kay Joy, our volunteer DCF Benefits Navigator; Renee Anderson, Case Manager and Southside Filling Station Program Manager; and Tricia McCourt, Case Manager and Substance Use Disorder Specialist—are here to bridge these gaps. We’re here to ensure that resources are not just available but also accessible.
Think of us as the ramp to the support you need. We guide individuals through the complexities of various forms and applications, ensuring that paperwork isn’t a barrier to progress. Our proactive case management approach helps our clients overcome the challenges that stand between them and their next steps to success.
With a person-centered focus, we take time to understand each individual’s unique needs and goals, providing tailored support to navigate the systems and processes required to access critical resources. We help people identify their goals and walk alongside them as they work to achieve those goals.
Tricia McCourt, one of our case managers, has been working with one man for about nine months who had experienced frustration and discouragement after repeatedly falling through the cracks due to his mental health, physical health, and a traumatic brain injury that caused significant memory issues. Through their work together, they have achieved several goals, including connecting him to mental health therapy and Vocational Rehabilitation. He has made great strides in overcoming barriers and attributes his success, in part, to having a case manager who hasn’t given up on him.
Whether it’s helping a family navigate DCF benefits, connecting someone to mental health services, or supporting those on the path to recovery, our mission remains the same: to make the path to a better future as clear and accessible as possible.
